Seeking a medical report from a GP/Specialist on an employee (long term absence)

This letter to a GP or Specialist asks for their expert opinion on the employee's state of health where the employee is on long term absence. If the employee is on short term, or a series of apparantly unrelated absences, please use the short term absence letter. This letter asks the Doctor to advise you on the employee's ability to return to work, or if still attending, what tasks they should avoid, and asks them to advise you if the employee is disabled under the terms of the Disabillity Discrimination Act and if so, what adjustments could be made to improve the employee's ability to work. Doctors are entitled to charge for providing a medical report and this letter confirms that you, as an employer are aware of this.
