Requesting an employee's consent to seek a medical report
This letter seeks an employee's consent for the employer to get a medical report on their condition from their GP or specialist. It includes a form that advises them of their rights under the Access to Medical Reports Act 1988.
Once you have gained the employee's consent, please see letter to GP/specialist (either long term or short term absence.)
