Useful Forms
Many of these forms are included within the relevant Policies and Procedures. Where there are Word/pdf attachments users can download a supply of forms. Word users are able, if necessary, to amend them to reflect their particular requirements. Unfortunatley, due to the nature of pdfs, they cannot be amended.
If you do decide to make changes to the forms, we must advise you that by amending them YOU DO SO ONLY ON THE BASIS THAT YOU ACCEPT RESPONSIBILITY FOR THE CHANGES THAT YOU MAKE! Absence & Lateness Record
This form allows you to keep an individual record of attendance for each employee
Adoption Leave Notification Form
An employee would use this form to advise you of their intention to take adoption leave. The form is also included in the Adoption Leave Policy and Procedure.
Disciplinary Record
This form allows you to keep a summary of all disciplinary action taken against an employee.
Emergency/Bereavement Leave Form
Employees must complete this form if they wish to, or have already, taken Emergency or Bereavement Leave. This form is included in the Time Off for Family Emergencies Policy and Procedure.
Employment Application Form
This is a basic recruitment application form for shortlisting purposes, last updated April 2007 to take account of possible age discrimination implications.
Flexible Retirement Request Form
Employees can complete this form if they wish to request an extension to their retirement date. This form is included in the Flexible Retirement Policy and Procedure.
Flexible Working Request Form
Employees who wish to request a flexible working arrangement must complete this form. This form is included in the Flexible Working Policy and Procedure.

Maternity Notification Form
This form is used by pregnant employees to advise their employer that they are pregnant and wish to take maternity leave. This form is included in the Maternity Policy and Procedure.
Medical Report Consent Form
This form is used to get an employee's consent to you asking their GP or specialist for a report on their health. This form is included with the letter to an employee seeking consent for a medical report.
Parental Leave Request Form
Employees can use this form to advise you that they wish to take Parental Leave. This form is included in the Parental Leave Policy and Procedure.
Paternity Leave Notification Form
Employees would use this form to advise you of their requirement to take Paternity Leave. This form is included in the Paternity Leave Policy and Procedure.
Return to Work Interview Form
This interview form provides the basis for a detailed and accurate discussion following an employee's return to work from absence.
Risk Assessment Form
This forms allows you to carry out a systematic assessment of identified hazards, and to report on measures recommended to reduce risk.
Self Certification Form
This form must be completed by employees who have been absent from work as a result of illness for less than 7 calender days.
Training Record
This form is a simple record of all training an employee has undertaken.
Working Time Waiver Form
The working time waiver form allows your employees to opt out of the 48 hour average working week, i.e. to work more hours.
