This interview form provides the basis for a detailed and accurate discussion following an employee's return to work from absence.

Interviews such as this are particularly useful in tackling short term unrelated absences that can cause real problems for a business. They highlight to employees that absence is being monitored and allows managers to manage the situation as appropriate.
